The following forms are required at your first session. Please read and complete upon your arrival or print and bring the Registration and Consent forms with you.

Client Registration Form (complete)	Client Information- Address, Phone, Email, and Insurance.
Notice of Privacy Practices (read only)	Private Health Information Uses and Disclosures with or without authorization, Client Rights, Counselor Duties, and Complaint Procedures.
Professional Disclosure Statement and Client Consent (signature only)	Kirkland and Ritchie Education and Experience, Counseling Goals, Confidentiality, Cancellations, Fees and Payment, and Consent Signature.
